Chelsea reached their third Champions League final in five seasons as they beat Real Madrid 2-0 at Stamford Bridge to set up an all-English showpiece against Manchester City in Istanbul on May 29.
After a goalless first leg in Madrid last week, Timo Werner fired Chelsea ahead in the 28th minute with his first goal since February.
Mason Mount doubled the lead just before half-time to leave Real with a mountain to climb in the second half.
Real pushed for a goal after the break but Chelsea defended resolutely and could have extended their lead late on.
The victory means Chelsea have now won six of their last seven matches against Real in all competitions, including two Champions League semi-finals.
It is also the first time since 2008 that two English clubs have reached the Champions League final.
